Je trouve que pour les accidents du style fracture tout dépend de comment ça a été fait...si c'est par négligence pour moi c'est pas un vrai accident.
Pour les films récents et morts/tortures pas "accidentelles" j'ai quelques "nouveau" liens que j'ai pas encore cités:

-Hobbit 2012 : 27 morts voici des explications de pratiques et de sécurité douteuses :
"A horse named Shanghai was hobbled (his legs were tied together so that he couldn’t move) and left on the ground for three hours because he was too energetic for his rider.
